DigitalDoctors@Home is a near-virtual healthcare platform that delivers in-person (to your living room), remote/telemedicine medical consultations, and follow up, chronic care management, and wellness support directly to patients in their homes using high touch, and high-tech capabilities, e.g., predictive analytics and AI.
How does DigitalDoctors@Home work?
We blend technology with personal care. Our care team uses easy-to-use monitoring devices, virtual visits, and home check-ins to keep track of your health, catch issues early, and make sure you get the right care—without the hassle of traveling to a clinic or hospital.

What services does DigitalDoctors@Home offer?
The platform provides primary care, urgent care consultations, chronic disease management, mental health support, post-discharge monitoring/care, and remote diagnostics using connected medical devices.
What conditions can be treated through DigitalDoctors@Home?
Common conditions include:

Does the platform offer 24/7 care?
Yes, many services—such as urgent care consultations and symptom assessments—will become available 24/7 with the growth of the company, through AI triage or on-call clinicians.
Can DigitalDoctors@Home replace in-person visits?
No.
Every person receives at least an annual in-person visit, followed by regularly scheduled telemedicine visits, supplemented as needed, in-home care by a nurse, Occupational and/or Physical therapist.
We’ll also include, if available in a community, in-home: podiatry, dental hygiene, blood drawing, diagnostics, etc.
What we can’t bring to the living room we will attempt to bring it to you virtually e.g., behavioral health, etc. Technology and Data Security
What is Remote Patient Monitoring?
Remote Patient Monitoring means you use simple devices at home—like a blood pressure cuff or scale—so we can check your health routinely. This keeps us connected to your wellbeing and allows us to step in quickly if something changes.
What is Chronic Care Management?
Chronic Care Management is ongoing support to help you manage long-term health conditions like diabetes or high blood pressure. Our team checks in regularly, helps you stick to your care plan, manages medications, and offers advice to help you feel your best.
Accessing Care
How do patients access care through DigitalDoctors@Home?
Patients can access care via a secure mobile app or web portal. Services may include in-person clinicians, video consultations, real-time monitoring, messaging with healthcare providers, and integration with wearable devices.
Who are the providers on the platform?
DigitalDoctors@Home collaborates with licensed physicians, nurse practitioners, mental health professionals, care coordinators, social workers, Community Healthcare Workers, and many other healthcare professionals to deliver holistic, patient-centered care
Is DigitalDoctors@Home available nationwide?
Availability depends on the availability of local providers in a community, telehealth regulations and licensure; however our platform aims to serve patients across multiple states through a growing network of licensed providers to deliver care and clinical support.
Can DigitalDoctors@Home replace in-person visits?
No.

Yes, the platform supports remote patient monitoring (RPM) through Bluetooth-enabled devices like blood pressure cuffs, glucometers, pulse oximeters, and more.
How does technology help my care?

Is my health information safe?

Payment & Insurance
How is DigitalDoctors@Home paid for under the fee-for service (FFS) model?
Under FFS, providers bill insurers (or patients directly) for each service provided, such as in-home care, virtual consultation, RPM session, or care coordination call.
What is the value-based care (VBC) model used by DigitalDoctors@Home?
In VBC, providers are reimbursed based on patient outcomes and cost savings rather than volume.
This includes bundled payments, shared savings models, and performance-based incentives for improving chronic condition outcomes or reducing hospital readmissions.
Can patients use insurance for services?
Most insurance plans, including Medicare and some Medicaid programs, may cover eligible some or all of the services.
Payment is determined by the coverage you have.
